PSYCHIATRIC PRESCRIBERS​

Kelly Lawrence, LPC, CRNP, PMHNP-BC is a Board Certified Psychiatric Nurse Practitioner with certification Across the Lifespan. Kelly provides psychiatric evaluation and medication management services to individuals age 5-65+. She received her BSN in 2020 and MSN in 2023 from Wilkes University. She holds an active license in Pennsylvania as a CRNP with prescriptive authority. Kelly uses a holistic, person- centered, strengths-based approach with her clients and keeps up-to-date on evidence-based treatments to insure her clients are receiving exceptional care. She individualizes care to the needs of each person she meets and offers a safe, comfortable, and professional experience. Before furthering her education, Kelly practiced as a licensed professional counselor since 2005. She received her undergraduate degree in Psychology from Carnegie Mellon University in 2000 and her graduate degree in Counseling Psychology from Boston College in 2002. Throughout her career she has helped others in various capacities including addictions treatment, partial hospital care, intensive outpatient treatment, wrap-around services, program coordination and outpatient services. Since 2008, Kelly has been in private practice serving the lake region and became the Owner of Arbor Vitae Behavioral Health in 2018.

Tracy Rockefeller, CRNP, ANP-BC, a compassionate Nurse Practitioner and Persian Gulf veteran who has dedicated over 20 years to clinical care in psychiatry. Tracy’s journey began with military service, and she utilized the GI bill to pursue her passion for medicine, specializing in inpatient, outpatient, and geriatric psychiatry. Her experience and commitment to patient-centered care ensure that each individual receives personalized treatment that meets their unique needs. Tracy is proud to serve the Lakeville community, offering expertise grounded in both military discipline and clinical compassion.

Dillon Yeager, CRNP, PMHNP-BC, is a Board-Certified Psychiatric Mental Health Nurse Practitioner who brings a compassionate, patient-centered approach to every individual he serves. His career began in the fast-paced environment of the emergency room, where he quickly discovered a deep passion for psychiatric care. This pivotal experience led him to pursue advanced training as a PMHNP and solidified his commitment supporting individuals on their mental health journeys.
With experience treating both children and adults across a wide range of psychiatric conditions, Dillon blends evidence-based psychiatric medication management with holistic therapeutic approaches. He is also well-versed in talk therapy and is dedicated to creating individualized treatment plans tailored to each patient’s unique needs and goals. Through this comprehensive and thoughtful approach, Dillon strives to empower his patients and help them achieve meaningful, lasting wellness.
In his down time, Dillon loves supporting his Philadelphia sports teams, adding a lively and good-natured rivalry with the New York sports fans here at Arbor Vitae—we always enjoy the banter.

Zechariah Howard, CRNP, PMHNP-BC is a Board-Certified Psychiatric-Mental Health Nurse Practitioner who is deeply committed to the field of mental health. His background in nursing has led him to care for psychiatric patients in emergency department settings, in acute inpatient units, and in outpatient settings during his PMHNP training, giving him a comprehensive and well-rounded clinical foundation. Several of his clinical rotations were completed here at Arbor Vitae Behavioral Health, providing him with valuable experience and insight into the practice and the community it serves.
Zechariah provides comprehensive psychiatric evaluations and medication management services across the lifespan, with a particular passion for supporting children and adolescents during their formative years. He is dedicated to delivering compassionate, evidence-based, and patient-centered care tailored to each individual’s needs. Zechariah is excited to join the team at AVBH and looks forward to making a meaningful and lasting impact within the community.
THERAPISTS
Baylee Johnson, MA, LAPC is a Licensed Associate Professional Counselor holding a Bachelor's degree in Human Services, with a focus on addiction from Lackawanna College and a Master's degree in Clinical Mental Health Counseling from
Sacred Heart University. While relatively new to the counseling profession, Baylee brings substantial life experience to her role and is driven by a profound desire to assist others, inspired by the support she received from her own therapist. Baylee has experience providing individual and group counseling services through her role at an addictions outpatient center and as a crisis counselor. Here at Arbor Vitae, Baylee has developed an interest in working with children, adults, families and couples. She excels at developing a supportive therapeutic relationship with her clients and provides a safe, supportive atmosphere for anyone she works with.
Outside her professional journey, Baylee cherishes moments with her loved ones, finding joy in the company of her rescue dog, Rusty. Eager to learn and grow, Baylee approaches her work with an open mind and a genuine enthusiasm for embracing feedback. Her commitment to making a positive impact shines through, making her a valuable addition to our team.

Chad Anuszewski, LPC has been a Licensed Professional Counselor (PC005293) in the State of Pennsylvania since 2009 following his graduation from the University of Scranton, earning a Master of Science Degree in Community Counseling in 2007. Chad is privileged to have joined Arbor Vitae Behavioral Health to continue practicing his chosen profession. Chad is credited with building a private counseling practice in 2009 until other professional ventures, including the creation of a Substance Use Disorder Treatment facility took him down a new pathway. Chad has also enjoyed being an Adjunct Professor for the University of Scranton from 2010 to 2014, helping educate, support and shape future counseling students achieve their own dreams of becoming a professional counselor. Chad Anuszewski specializes in Dialectical Behavioral Therapy (DBT), helping his clients identify, build, and achieve coping skills that create an individual’s ability to overcome behavioral health issues, leading to a happy and fulfilling life. More recently, Chad has been working with young athletes develop a healthier, confident mindset to prosper in their chosen sport. This has also translated into academics, personal, professional, and daily life activities.
In his spare time, Chad enjoys spending time with his family (Wife, Maria, Daughter, Emma and
Son, Jonathan). Chad also enjoys spending time in the outdoors, fly fishing, coaching baseball,
and taking hikes with his two dogs, Rocky and Marley.

Elaine Rogai, MA earned her Bachelor of Science in Psychology from Marywood University and her Master’s degree in Clinical Mental Health Counseling from Walden University. She is a lifelong resident of the area with more than 20 years of experience in social services, including case management, medical social work, and housing coordination throughout Wayne and Pike Counties.
Elaine is passionate about being part of a team dedicated to providing exceptional service to the community. She is known for her warm, open, and accepting approach, and she utilizes a person-centered framework to support individuals navigating a wide range of challenges. Her work is grounded in compassion, cultural competence, and sensitivity to the diverse needs of those she serves.
Drawing from her own lived experience with mental health challenges and recovery, Elaine brings authentic empathy and understanding to her work with clients.
Outside of the office, Elaine enjoys spending time with her family and engaging in creative pursuits such as crocheting, painting, and music.

Toby Dennert, MPS, LPC, graduated from the University of Minnesota with a Master of
Professional Studies in Integrated Behavioral Health. He has been licensed in
Minnesota as an alcohol and drug counselor since 2019 and as a Licensed
Professional Counselor since 2021, before relocating and joining Arbor Vitae. Toby brings a wide range of experience across residential treatments, outpatient therapy,
and in-home services, using a collaborative approach to help clients gain insight and
create lasting change.
Toby tailors his work to each individual he works with, drawing from multiple modalities including cognitive behavioral therapy, solution-focused therapy, accelerated resolution therapy, and polyvagal-informed approaches. Together with clients, he focuses on setting clear, realistic goals that are broken into manageable steps in order to create progress that feels meaningful, sustainable, and aligned with each client’s values. Toby works with a variety of presenting concerns, including substance use, life transitions, relationship challenges, and trauma.
Outside of his work, Toby can often be found slowing life down with a good book, listening to a podcast on a walk, or cheering on his beloved Chicago Bears, fueled by a combination of unyielding loyalty and perpetual optimism.

Sarah - Administrative Assistant Sarah joined Arbor Vitae as an Administrative Assistant in December of 2024. She is honored to play a part in providing such vital services to the area, and she is passionate about creating a safe space where every member of the community feels welcome. Sarah enjoys reading, writing, and antiquing with friends and family. She can spend all
day thrifting watching tennis, or hanging out with her cat!

Amanda - Administrative Assistant Amanda is pleased to join the Arbor Vitae Behavioral Health team, where she brings a compassionate, patient-centered approach to care. She is deeply committed to supporting individuals throughout their mental health journey and prioritizes empathy, dedication, and respect in every interaction. Outside of work, Amanda enjoys spending quality time with her family, relaxing with her cats, and making the most of her free time through tubing adventures and attending live concerts.

Amanda - Front Desk Associate is the welcoming face of our office, serving as our front desk receptionist. She is committed to creating a warm, supportive, and organized environment where every client feels valued from the moment they walk through the door or call the office. Amanda understands that reaching out for support can be vulnerable step and strives to make each interaction comfortable, respectful, and reassuring. She is passionate about mental health awareness and is dedicated to supporting the individuals and families we serve with compassion and professionalism. Amanda grew up in Florida and recently relocated to be closer to family and begin a new chapter. Outside of work, she enjoys taking walks with her new fur sister, Dakota, listening to music, and embracing the beauty of the changing seasons.
